Operates as a program under the Healing and Wellness Branch

• Provides information, advocacy and support to assist frail, vulnerable and at-risk elderly adults and individuals with care needs to remain in their homes as long as possible
• Acts as respite for informal caregivers such as extended family

Community Support Services offered include but are not limited to:

• Caregiver support
• Case management
• Friendly visiting
• Service arrangement and coordination
• Transportation for medical appointments within Sudbury and/or assistance with medical appointment arrangements
• Visiting for social and safety reasons, either in person or on the phone

Provides health care services for ill, injured or elderly community members to allow them to remain in their own homes

Provides the following services:

• Assessment to determine individuals health history and develop a personalized care plan
• Case management to ensure that care plans are being carried out and that the plan developed is still suitable
• Home care through Personal Support workers
• Personal care help with activities such as dressing and bathing
• Basic foot care
• Teaching the individual in need to care for themselves
• Companionship
• Home support with light housekeeping, laundry and meal preparation
• Respite for family members and caregivers

Provides a variety of non-medical home support care services including the following:

• Accompaniment to various appointments and events (doctor, religious and cultural events, shopping)
• Homemaking services such as cleaning, laundry, meal preparation, home care, snow removal and yard work
• In-home visits and companionship
• Personal care assistance such as bathing, dressing, dental hygiene, grooming and help getting in and out of bed
• Relief/respite for caregivers on an as-needed basis

Provides a variety of home-based nursing services, including:

• Chronic Disease Management
• End of Life Care
• Foot Care
• Intravenous Care
• Medication Administering
• Pain Management
• Serious Injury Care
• Ventilator Care
• Wound Care

Offers 24-hour nursing services with personal emergency alert systems available

Offers medically based services, including:

• Catheter care
• Foot care
• Medication administration
• Monitoring of vital signs
• Palliative care
• Wound dressing and care

Formerly Community Care Access Centre

• Assists clients to plan and make informed choices about managing their health care needs or those of someone you care for
• Coordinates information about other services available within the community and link you with these services when appropriate
• Determines access to health care services provided in the home
• Determines admission to long term care facilities (homes for the aged, nursing homes)
• Determines access to health support services provided in schools for children with special needs
• Maintains an ongoing waiting list for placement, programs and services

Provides on-going or short-term support services to assist elders to remain in their homes as long as possible

• Provides assistance with household chores, light housekeeping, laundry, meal preparation, and needs in the home as they arise
• Provides household support for heavy and seasonal housecleaning as well as light home maintenance work
• Acts as respite for informal caregivers such as extended family
• Acts as a social support for isolated or ill individuals

Provides health care services for ill, injured or elderly community members in their own homes to allow them to live independently for as long as possibleProvides the following services through a Case Manager/Nurse, Personal Support Workers and Homemakers:

• Assessment to determine individuals health history and develop a personalized care plan

• Offers a variety of in home care/nursing services and respite services provided by Registered Nurses, Registered Practical Nurses, Personal Support Workers and Homemakers
• Provides free in home assessments

• Escorting to appointments

• Health and influenza clinics
• Home support including cleaning and laundry

Registered Nursing staff assist with:

• Administering medications
• Assessing the needs of each individual
• Caregiver respite

Offers a variety of services to families whose children have cancer, including:

• Providing healthcare and health education
• Assistance navigating the health care system, and locating and accessing community and hospital resources
• Working with schools and communities to build support systems for children with cancer

• Operates a residential hospice with 10 private bedrooms and shared common areas
• Provides 24-hour nursing and personal support care
• Offers compassionate bedside support, including social and emotional support, to individuals (regardless of their age, disease or religion) at home, in hospital, in long-term care facilities or in a residential hospice

Offers Grief and Bereavement Support groups to residents, families, staff and volunteers:

• Open Grief Discussion Groups on a monthly basis
• Closed Grief Recovery Programs - 8 week program offered twice a year
• Individual Grief Coaching

Volunteer Hospice Service

• Offers non medical palliative volunteer support to persons living with a terminal illness and their family
• Visits in home or hospital or other facility
• Supports patients emotionally, spiritually and practically
• Provides respite to caregivers and bereavement support

Breastfeeding Support

• Offers support for mothers and mothers-to-be who may have concerns or difficulties with breastfeeding

Community Wellness Clinic

• Offers full primary health care
• Provides treatment of minor acute illnesses

Corporate Health Services

• Offers occupational health programs and services to meet the needs of employees

Visiting Nursing Services

• Provides in home nursing services for health care issues, including foot care

Friendly Visiting

• Offers regular visits between a volunteer and a homebound person to provide friendship and companionship

Hospice Volunteer Visiting

• Provides support during the final stages of illness

Life Line Program

• Provides peace of mind for clients and their families
• Provides quick assistance whenever personal support is needed

Nursing and Nurse Practitioner Services

• Provides nursing services with the Nurse Practitioner Clinic to individuals in their school, workplace or clinic setting

Respite

• Offers respite to caregivers of elderly frail or disabled individuals one weekend a month

Supportive Housing

• Provides assistance to older and physically disabled adult who require assistance to live independently in the community

Transportation Service

• Provides transportation to local and some out-of-town medical appointments
