• Provides support for Indigenous people in need of non-hospital care who are frail, vulnerable and at risk
• Meets the physical, mental, emotional and spiritual needs of people who require care, support, information, service referrals, health education or extended support

Services offered include:

• Medical transportation services
• Medical escorts
• Referrals to community services and Friendship Centre programs
• Preparation of forms and applications
• Congregate dining services
• Home visits/friendly visitation
• Security checks (in person or over the phone)
• Caregiver support services (respite and training)
• Advocacy with community agencies
• Access to cultural activities, traditional foods, ceremonies, Elders and Healers
• Transitioning from ODSP to Old Age pension
• Locating a family doctor
• Assistance in obtaining medical equipment and supplies

Responsible for processing Non-Insured Health Benefits Service claims and coordinating medical transportation for community members

• Provides transportation to medical appointments within the community for members without access to a vehicle
• Provides transportation and/or assistance from/to Aundeck Omni Kaning First Nation to medical appointments (doctor, dentist, optometrist, specialist) in Little Current, Espanola and surrounding area
• Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

NOTE: Individuals with small children must have appropriate car seat restraints or transportation will not be provided

Responsible for coordinating medical transportation for community members

• Provides transportation to medical appointments within the community for members without access to a vehicle
• Provides transportation and/or assistance from/to Wikwemikong to out-of-town medical appointments (doctor, dentist, optometrist, specialist) in Little Current and surrounding area
• Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

• Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
• Assists with making travel arrangements for long distance medical appointments
• Assists eligible clients with local medical trips to North Bay, Sturgeon Falls and Sudbury
• Maintains client documentation, receipts and related information

• Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

Provides transportation services to individuals who are 55 years of age or older and/or who have a physical or mental disability

Operates as a program under the Healing and Wellness Branch

• Provides information, advocacy and support to assist frail, vulnerable and at-risk elderly adults and individuals with care needs to remain in their homes as long as possible
• Acts as respite for informal caregivers such as extended family

Community Support Services offered include but are not limited to:

• Caregiver support
• Case management
• Friendly visiting
• Service arrangement and coordination
• Transportation for medical appointments within Sudbury and/or assistance with medical appointment arrangements
• Visiting for social and safety reasons, either in person or on the phone

Delivers medical transportation services and assistance in making medical appointment arrangements

• Provides transportation to medical appointments within the community
• Provides transportation from/to Whitefish Lake First Nation Reserve to any medical appointments (doctor, dentist, optometrist, specialist) outside the community in the Sudbury area
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

Provides community health services and assistance with non-insured health benefits to eligible First Nation and Inuit people

Non-Insured Health Benefits
Provides assistance with accessing and making arrangements for extended health benefits, including:

• Dental care
• Medical transportation
• Medical equipment
• Medications
• Mental health counselling
• Short-term crisis intervention
• Vision care

Provides affordable door to door transportation for seniors or those with a disability to help them with various activities such as grocery shopping, medical appointments and social outings

NOTE: Vehicles are not equipped to accommodate wheelchairs

Offers assistance to patients who are transitioning from a hospital stay to home which may include the following:

• Assistance with transportation
• Home configuration
• Shopping
• Finding other community support services to help

• Offers a low cost, door-to-door transportation service for people with disabilities that prevent them from using the conventional transit system
• Provides specialized accessible buses that include low floors to replace steps, automated wheelchair ramps, fold up seats for mobility devices, auditory stop call-out and visual display screens, and priority seating

Services can be used for the following trip purposes:

• Work
• School
• Medical
• Other outings such as shopping, social, or church

Provides greater access to public transportation by offering transportation to and from the nearest public transit stop to less populated areas of the city
