• Assists individuals in creating a personalized fitness program and provides supervision while participant performs the program

Healthy Eating, Healthy Weight

• Offers a weight management program
• Provides encouragement and health information to support individual in achieving wellness goals

Smoke Free Living/Smoking Cessation

• Provides education and resources about initiatives to quit smoking
• Offers a support group including mentors who have gone through the process of smoking cessation

Youth Leadership

• Provides workshops to teach youth leadership skill development
• Addresses topics including conflict management, decision making, goal setting, teamwork, time management and developing and presenting workshops

Allows individuals experiencing drug or alcohol addictions to receive counselling, appropriate addiction medications, and connections to community treatment programs for ongoing montioring, support and rehabilitation

Services available include:

• Access to medication assisted therapy for alcohol and opioid use disorders
• Addiction medicine which includes opioid replacement therapy buprenorphine (suboxone) and methadone
• Alcohol urge reduction medication
• Brief support and referrals for individuals not connected to addiction recovery services

Offers a methadone maintenance program for opiate dependence based on an intensive outpatient model, over a 12-24 month period

Services include:

• Initial intake screening procedure
• A signed treatment contract
• Focused medical examination
• Appropriate lab tests
• Initial as well as ongoing supervised urinalysis
• Weekly group and individual therapy
• Regular medical follow-ups and supervision
• Regular daily Methadone dosing
• Treatment of substance use related illness and/or diseases (such as Hepatitis C and HIV infection)
• After care program

• Needle Exchange Program
• Naloxone Overdose Response Program - a lifesaving medication that can quickly restore the breathing of a person who has overdosed on heroin or prescription opioid pain medication

• Offers a 12 week group education and counseling program on domestic violence and non-abusive ways of resolving conflict
• Offers victims/partners of the offender support including safety planning, referrals to community resources and information on offender's progress

Problem Gambling

• Provides individual and family counseling to deal with the harmful effects of problem gambling

• Functions as a withdrawal management and problematic alcohol/substance use treatment centre
• Offers a 28-day land-based program in an eight-bed co-ed residential treatment facility (also offers two detoxification beds)
• Addresses the needs of an individual's spiritual, mental, physical, and emotional health and well-being
• Assists individual to gain an understanding of how abuse, relationship dynamics, problematic alcohol and substance use issues, self-destructive behavior and family violence affects their life
• Supports individual through a journey of self and includes components such as detoxification, stabilization, introductions to food, exercise, and cultural activities and healing practices

Services and programs include but are not limited to:

• Assessment and treatment planning
• Medically-supervised detoxification
• Individual/group counselling
• Nutrition information and guidance
• Cultural/recreational activities such as fishing, trapping, snowshoeing, and medicine picking

Provides a 9-bed residential treatment facility and support services for adolescents with alcohol and substance use difficulties combined with mental health or behavioural issues

• Offers a 90-day substance use treatment program with a focus on culturally relevant programs, counselling and therapy
• Operates an on site school with qualified teaching staff to allow residents of the Centre to continue with academic studies
• Coordinates cultural naming ceremonies, feasts, sweat lodges and circles through the supervision of traditional elders
• Promotes greater responsibility and involvement of the family members in the adolescent's life

Provides education and counselling focused on:

• Developing a balance between the emotional, spiritual, mental, physical and social aspects of an individual's life
• An individual's identity, belonging and sense of direction in life
• Developing a healthy lifestyle, free of alcohol and substance use
• Teaching relevant life skills

Aftercare Program

• Provides a referral to another clinician, elder or service provider three weeks prior to discharge who will continue to provide service for 30 days after discharge
• Offers an aftercare support group once per week for as long as needed
• Offers continued access to cultural ceremonies

• Designated child welfare beds at the treatment facility for First Nation's communities requiring child placement

Intervention

Aftercare provided for preventing reoccurrence:

• Counselling, sharing circles and support groups
• Referrals to band services, social services, detoxification and treatment centres and medical assistance

• Promotes the health and well being of urban Indigenous people
• Focus is on preventing the development of health issues including diabetes, obesity and heart disease

Programs and services provided include:

• Sports activities, physical fitness and recreational activities
• Helping individuals in creating a personalized fitness program and supervision while participants perform the program
• Healthy nutrition education
• Smoking cessation/smoke-free living education and programs

• Offers the "Stop Smoking with FHT's" cessation program to assist individuals in quitting tobacco use
• Conducts initial assessment to determine the patient's smoking habits and health profile to provide the appropriate treatment
• Provides self-management support and strategies
• Assists patients released from hospital to remain smoke-free after discharge

• Counselling
• Nicotine Replacement Therapy (NRT) that can include patch, lozenges, gum and inhalers
• Prescriptions for Champix or Zyban (through FHT Pharmacist)

• Offers opioid agonist therapy (methadone or Suboxone) for individuals overcoming opiate dependence (heroin, oxycontin, morphine, fentanyl, etc)
• Provides an intensive outpatient harm reduction program designed to reduce risk to ones self and society, maintain stable lifestyle, eliminate withdrawal symptoms, improve self esteem, health and overall quality of life

• Offers sign-up forums to become involved in ways to quit smoking and to fight tobacco
• Offers support and guidance to any youth desiring to become involved in health advocacy issues/public health programs

• Provides facts and figures about the movie industry's endorsement of smoking
• Provides online avenues for youth to become involved in the fight against smoking appearing in movies
