• Oversees various types of housing options available to individuals with intellectual and/or developmental disabilities
• Provides supports to individuals living in the homes as determined by individual needs; supports can range from a few hours to 24-hour supports with a focus on assisting the individuals to gain the highest possible level of independence and community inclusion

Offers supports to individuals with developmental disabilities living in their own apartments within a supervised setting

• Supports can vary from a few hours to 24 hours per day, depending on the individual's needs
• Aims to meet all aspects of the individuals needs, including but not limited to personal hygiene, cooking, cleaning, socialization, financial management and health
• Provides one to one personal support within the home or community
• Teaches and/or maintains daily living skills, problem solving skills
• Determines the degrees and types of supports required by the needs of the individual receiving supports and their families

Specialized Service Coordination (SSC) program

• Uses a person-directed approach to help individuals identify their goals and the steps to take to reach their goals
• Assists people with direct funding to learn how to create their own support networks within their available funding

Adult Protective Service Worker (APSW) program

• Supports individuals who are living on their own to live as independently, safely, and securely as possible in the community
• Assists in developing strategies for coping and adapting to life in the community
• Ensures that individuals are informed of their rights and are treated with respect, fairness and dignity (Advocacy)
• Assists individuals in managing income support from Ontario Disability Support Program (ODSP) (Temporary Trusteeship)
• Helps individuals use natural supports as a way of enhancing lives and understand the range of opportunities that exist in the community

APSW program also provides services in Marathon at 115 - 52 Peninsula Square

Local drop-in day program that provides a variety of weekly interactive social and life skills activities in a safe and accessible environment for individuals living with disabilities

• Offers children and adult athletes year-round training and competition in many Olympic-type sports at Invitational and Regional meets

Operates a 100-unit seniors' supportive housing building

• Owned by the District of Thunder Bay Social Services Administration Board (TBDSSAB) who manages building rentals
• Offers 24-hour on-site non-medical support services to residents, supported by the City of Thunder Bay

Provides programs and services to meet the needs of individuals with an intellectual and/or developmental disability

Transitions Program

• Provides support to youth with developmental disabilities to create a life plan for the transition to life after graduating from high school
• Assists with various applications such as housing, passport, special services and Ontario Disability Support Program
• Offers guidance for accessing further education, employment and training opportunities and community resources
• Advocates on behalf of the family and individual

Provides education and training for individuals to secure employment and to complete work contracts

• Offers assistance and guidance to learn skills required to meet job requirements for both on/off site employment opportunities
• Provides supervision and training at the various job sites until individual is able to perform job independently
• Provides transportation to/from places of employment

Supported Employment

• Provides supports to businesses that employ individuals with developmental and/or intellectual disabilities
• Provides training to the employed individual

Coordinates leisure and learning activities, social outings and assists in securing employment opportunities

• Provides emotional and social support
• Acts as an advocate for individuals and their families
• Provides person centered planning and assistance in developing personal support agreements between individuals, families and Ministry of Children, Community and Social Services (MCCSS)

Vocational Program

• Provides life skills training
• Coordinates and supervises leisure and recreational activities
• Offers supervised and unsupervised volunteer opportunities

• Provides services and supports to meet the needs of adults with an intellectual and/or developmental disability
• Works on a "person-centered" model to address the values, priorities and preferences of each individual

Provides the following programs and services to enhance individual growth and independence:

• Provides support services for Passport recipients
• Supervises a Training and Leisure Centre which coordinates leisure activities and supported employment opportunities
• Offers individualized supports through supported independent living apartments

Provides residential services and supports for adults with an intellectual and/or developmental disability

• Offers individualized residential support services in individual apartments or person's home
• Aims to meet all aspects of the individuals needs, including but not limited to personal hygiene, cooking, cleaning, socialization, financial management and health
• Provides one to one personal support within the home or community
• Supports required are determined by the individual receiving supports and their families

Provides rent-geared-to-income (RGI) supportive housing in landlord/tenant units for individuals with challenges

• Provides assistance in securing rent geared to income (RGI) housing units
• Operates under the Residential Tenancy Act
• Focuses on independent living with minimal assistance
• Provides 24-hour on-site support staff

Provides various housing services and supports for individuals with developmental disabilities

Supportive Housing

• Provides 24 hour supportive living accommodations
• Offers a few rent geared to income units
• Offers assistance with daily living skills including laundry, housework, cooking and personal care
• Provides accompaniment to appointments and social outings

Supported Independent Living

• Provides in home and out of home support in areas such as housecleaning, laundry, budgeting and grocery shopping to individuals living independently within the community
• Offers accompaniment to social outings and appointments if required

Enriched Supported Independent Living (ESIL)

• Offers support services in an individual's own home for those who do not require 24 hour support
• Supports vary with each person and can include budgeting, attending medical appointments, daily meal preparation, housekeeping and developing relationships

Intensive Supported Independent Living (ISIL)

• Uses the "Housing First" model for individuals with significant barriers to housing
• Provides supports for individuals who have difficulty securing and maintaining housing

Outreach Supported Independent Living (OSIL)

• Provides case management for individuals with complex needs including an intellectual disability, serious mental illness, and addiction
• Supports individuals with the transition from youth to adulthood

• Offers direct assistance for adults with a developmental disability who are living on their own
• Assists adults with an intellectual disability to acquire skills for daily living and to access any community supports and services

Provides the following services:

• Case management
• Assistance to coordinate community resources
• Development of service plans with other service providers
• Emotional support during stressful life changes
• Information about other available resources, supports and services

• Provides programs, services and supports for adults with an intellectual and/or developmental disability
• Provides education, information and resources on various mental health and developmental disability issues

Person Directed Planning (PDP)

• Identifies what individuals needs and wants are in relation to life in general (assistance from family requested)
• Identifies individual's personal, educational, vocational, medical, and safety support needs in relation to wants
• Connects and supports individuals in accessing resources to meet identified needs and wants of the individual

• Coordinates leisure and learning activities outside of the home environment to promote social experiences
• Provides a variety of settings for participation to help meet the goals or interests of the individual

Supports and activities can include but are not limited to:

• Age-related activities such as senior centre participation
• Educational and general interest activities such as cooking and sewing
• Sports and recreation supports such as fitness programs, swimming, camping and music groups
• Trips for shopping, sports events and concerts
• Volunteer opportunities such as animal shelters, community food banks and libraries

Offers supports to individuals with developmental disabilities living in their own homes

• Supports can vary from a few hours to 24 hours per day, depending on the individual's needs
• Aims to meet all aspects of the individuals needs, including but not limited to personal hygiene, cooking, cleaning, socialization, financial management and personal health care
• Provides one to one personal support within the home or community
• Teaches and/or maintains daily living skills and problem solving skills
• Degrees and types of supports provided are determined by the individual receiving supports, their families and service professionals such as Community Support Workers

• Supports individuals with a developmental disability who want to live in the community but require additional supports in order to maintain their independence
• Creates an individualized care plan based on a person's needs and goals
