Hospice Services - volunteer delivered one on one support for you and/or a loved one who has been diagnosed with a life limiting illness.

Palliative Care Nurse Navigator becomes your care partner and link to the health care team to ensure timely and ongoing implementation of shared care plans, assessment, referral, access to palliative care, grief, bereavement and support services.

The Palliative Care Community Team is a partnership of Haliburton Highlands Health Services, Haliburton Highlands Family Health Team, Central East Local Health Integration Network and Peterborough Hospice Palliative Pain and Symptom Management Consultation Services.

Drop-in Grief Support Groups are an informal, monthly get together for those who are experiencing bereavement. Offers a safe and non-judgement peer environment that can help you share your grief with others who feel the same way.

• An opportunity to visit with others who are also looking for comfort and community

Program is designed to offer a range of social, physical and recreational activities to meet the needs of frail, older adults, individuals with memory impairment and adults with physical disabilities.

• In addition to providing a safe, supportive environment for clients the program provides caregivers with respite from their caregiving role.

The Security and Reassurance Program provides quick daily phone calls to check in on clients to make sure all is well.

• Service is provided by volunteers, free of charge.
• Community Support Services enable seniors 55 and older and adults 18 and older with disabilities and/or illness and their caregivers to remain independent at their place of residence for a longer period.

Emergency Response Service provides clients and their loved ones with peace of mind in case of a medical emergency.

• An Emergency Response Unit and client pendant (necklace or wristband) allows clients to press a button when in a medical emergency and receive an immediate response from a 24/7 monitoring station that can determine next steps you require (ambulance, family member contacted).

Living with Cancer Support Group, open to anyone who has been diagnosed with or survived cancer.
