Community Paramedic Program


Description

Helps seniors and other patients live independently longer, and reduce emergency room visits and hospital admissions

Services may include:

  • Home visits and monitoring
  • Wellness clinics
  • Cognitive screening
  • Chronic disease management support and follow-up
  • Medication review

Community Paramedicine Remote Patient Monitoring (CPRM)

  • Monitors a patient's Chronic Obstructive Pulmonary Disease (COPD) or Chronic Health Failure (CHF) from the comfort of their home
  • Based on information provided by the patient and emailed to the local paramedics, the Paramedics may call to discuss the patient's health and provide a home visit, if need be (or transport the patient to the hospital if the condition is serious)
  • Paramedics may also refer the patient to other allied agencies such as Ontario Health atHome, Home Care, Diabetic Nursing, Meals on Wheels or the patient's own family doctor
  • CPRM does not replace the 911 Emergency System, but is another tool the Cochrane District EMS uses to help serve patients in the Cochrane District.
