Case Management
- Description
- Offers case management services to support individuals and families
- Works with individuals to help them identify their needs and goals
- Provides counselling services
- Teaches life skills, including budgeting and grocery shopping
- Refers individuals to other community services, if needed
- Advocates for individuals to ensure their access to income and housing
- Eligibility
- individuals 16 years of age and older who have more persistent or severe mental health symptoms, and who require more support with day-to-day activities, or require support in a number of areas of their lives such as housing, finances or physical health
- Individuals dealing with a family member's mental illness
- Application Process
- Completion of intake assessment required to determine individual's needs and goals
- Wellness and recovery plan is created with client centred on needs assessment
