Aging At Home Program
- Description
- Provides home support to Indigenous seniors so they can remain in their own homes as long as possible
- Provides education and information related to health and well-being
- Acts as a resource and respite for family members who are caring for an elderly loved one
- Assistance with personal care such as bathing
- Home support with light housekeeping, laundry and meal preparation
- Friendly visits to help with loneliness
- Assistance with coordination of contracted services for lawn care and snow removal
- Transportation/accompaniment to medical appointments and for errands such as banking and grocery shopping
- Eligibility
- Métis individuals 55 years of age and older in financial need and requiring support to remain living in their own home
- Application Process
- Referral required from Physician or Nurse Practitioner
- Intake assessment conducted with review of current home/family supports

- Referral form and completed intake assessment required
