Aging At Home Program


Description
  • Provides home support to Indigenous seniors so they can remain in their own homes as long as possible
  • Provides education and information related to health and well-being
  • Acts as a resource and respite for family members who are caring for an elderly loved one
Services provided can include but are not limited to the following:
  • Assistance with personal care such as bathing
  • Home support with light housekeeping, laundry and meal preparation
  • Friendly visits to help with loneliness
  • Assistance with coordination of contracted services for lawn care and snow removal
  • Transportation/accompaniment to medical appointments and for errands such as banking and grocery shopping
Eligibility
Métis individuals 55 years of age and older in financial need and requiring support to remain living in their own home
Application Process
  • Referral required from Physician or Nurse Practitioner
  • Intake assessment conducted with review of current home/family supports
