  • Provides support to youth with developmental disabilities to create a life plan for the transition to life after graduating from high school
  • Assists with various applications such as housing, passport, special services and Ontario Disability Support Program (ODSP)
  • Offers guidance for accessing further education, employment and training opportunities and community resources
  • Advocates on behalf of the family and individual
Eligibility
Individuals 18-25 years of age and older with an intellectual and/or developmental disability graduating from high school
