Interdisciplinary health professionals provide programs for patients including: Healthy eating: - Registered Dietitian / Certified Diabetes Educator offers nutrition counselling for a variety of health issues including: diabetes, pre-diabetes, cholesterol, hypertension, weight management, gastrointestinal health, osteoporosis, fatty liver, anemia, GERD, gout, and metabolic syndrome
Hypertension: - two practitioners trained through the Heart and Stroke Foundation provide assessment and ongoing support for hypertension management, and may provide 24-hour BP monitor for screening purposes
Diabetes Clinic: - provide counselling and guidance to manage Type 1, Type 2 and Gestational Diabetes, counselling for pre-diabetes, individual and group counselling available
Mental Health Counseling Program: - counselling services are available by referral via our two Registered Psychotherapists to rostered patients with the West Champlain Family Health Team
Memory Clinic: - comprehensive assessment of individuals who may be having some memory impairment including a full cognitive assessment, including driving safety, family support and resources are provided
Smoking Cessation (Quit Support): - provides education, support and counselling to individuals interested in quitting smoking or reducing the harm from smoking * options for treatment are detailed and free 'Nicotine Replacement Therapy' is available
