Life Long Care Program


Description
  • Provides programs and services to address needs of elderly Indigenous people and those with chronic illness and/or any form of disability
  • Offers advocacy support between individual and other agencies and organizations
  • Provides information and access to mainstream long-term care services such as the North East Local Health Integration Network (NELHIN)
Adult Day Program
  • Provides an afternoon for socialization
  • Teaches various craft making sessions with focus on traditional Native crafts
Friendly Visiting
  • Arranges regular visits from volunteer in individual's home for socialization
  • Provides emotional and physical support such as light housekeeping as required
Security Checks
  • Provides a phone call or visit to individual's home at mutually agreed time to ensure safety of home and/or individual
Transportation Program
  • Provides escorted transportation to medical appointments, shopping, and various social activities and programs
Eligibility
Urban Indigenous clients, regardless of age, who are disabled, chronically ill, frail elderly and/or require acute/chronic continuum of care
Application Process
Requires a signed consent form and completion of program intake form
