Integrated Care Solutions which offers a bundled care model and provides home care services to patients transitioning from hospital to home that can include:
  • Case management
  • Infusion pharmacy
  • Medical equipment and supplies
  • Nursing
  • Occupational therapy and physiotherapy
  • Personal support
  • Respiratory therapy
  • Speech and language therapy
Alternative Level of Care (ALC) Transitional Program which offers supports to patients who transition from a hospital to an interim short-stay facility and include:
  • Supports during the intensive rehabilitation recovery process
  • Collaboration with hospital, family and caregivers to prepare patient for moving back home
Government Funded Care Solutions for which Bayshore is a contracted health care provider for government-funded services for both bundled/integrated service delivery models and fee-for-service models, offering a range of services including:
  • Falls Prevention
  • Frail and Elderly Care
  • Medical Assistance in Dying (MAID)
  • Nursing
  • Paediatric Services
  • Palliative Care and Services
  • Personal Support Services
  • Physiotherapy, Occupational Therapy and Speech Pathology
  • Post Acute Community Infusions
  • Wound Care
