Men's After Care and Transition Program
- Description
- Provides accommodations and support to men who have completed a residential addictions treatment program
- Assists in recovery by offering an affordable, abstinent based, cooperative and empowering environment in close proximity to the Recovery Home facility for support
- Requires assistance with weekly chores and preparation of own food
- Eligibility
- Male youth and men 16 years of age and older who have completed a residential treatment program
- Application Process
- Requires completion of Referral form online OR a completed intake via phone with Assessment Coordinator and:
- Must have successfully completed a Residential Supportive Treatment program at Monarch Recovery or elsewhere and
- Must be abstinent from alcohol and drugs and be committed to recovery and
- Must have a source of income and
- Attend Community Aftercare program, and 12-Step meetings
