Northern Ontario - Hospital Transition to Home Program
- Description
- Offers assistance to patients who are transitioning from a hospital stay to home which may include the following:
- Assistance with transportation
- Home configuration
- Shopping
- Finding other community support services to help
- Eligibility
- Individuals 55 years and up who:
- are able to direct own care or caregiver
- are medically stable
- are able to be left unattended
- are mobile, ambulatory (able to self-transfer), and not need the assistance of more than one person to manage stairs
- pose no threat to the health and safety of self or workers AND
- do not live in a long-term care home or Assisted Living where 24-hour care is provided
- are able to direct own care or caregiver
- Application Process
- Potential clients are identified by the hospital or via Home and Community Care Support Services and bookings must be received a minimum of 24 hours in advance
