Northern Ontario - Hospital Transition to Home Program


Description
Offers assistance to patients who are transitioning from a hospital stay to home which may include the following:
  • Assistance with transportation
  • Home configuration
  • Shopping
  • Finding other community support services to help
Eligibility
Individuals 55 years and up who:
  • are able to direct own care or caregiver
  • are medically stable
  • are able to be left unattended
  • are mobile, ambulatory (able to self-transfer), and not need the assistance of more than one person to manage stairs
  • pose no threat to the health and safety of self or workers AND
  • do not live in a long-term care home or Assisted Living where 24-hour care is provided

Application Process
Potential clients are identified by the hospital or via Home and Community Care Support Services and bookings must be received a minimum of 24 hours in advance
