  • intake & elgibility
  • client assessment
  • case management
  • referrals to Algonquins of Pikwakanagan Home Care
  • referrals to Home and Community Care Support Services for: home nursing, occupational therapy, physiotherapy
  • provides patient advovcay as requested by clients for appointment, etc
  • maintains an equipment lending system for medical or equipment supplies on a short term basis for home and community care and home care clients
Eligibility
Must be a residing, registered status Algonquin of Pikwàkanagàn First Nation member. no age restriction. people living with a disability, chronic or acute illnesses, convalescing at home individuals, the elderly Residency Requirements: Algonquins of Pikwàkanagàn First Nation

Referrals can be made in person, by phone or in writing by client, caregivers or other health providers
