Description
Family-centred care program designed to assist those diagnosed with a terminal illness or a life limiting illness (chronic disease) * Support is provided to individuals and families in home, hospital or nursing home * Volunteers provide companionship, assistance with light household duties, and simple comfort measuresBereavement support and grief programs include one-on-one support, monthly caregiver-support groups, and ongoing social support groups. Any individual who has suffered a significant loss is welcome to bereavement support Respite service provided for family members who are caring for a love one * advanced care planning workshops and presentations available to groupsA visiting hospice - no beds provided
Eligibility
Living with life threatening or terminal illness defined as twelve (12) months or less, and includes their families/caregivers, and bereaved individual Live within the geographic boundaries of the District of West Parry Sound

Residency Requirements: North to French River, South to MacTier, east to Dunchurch
