Description
Starting July 8, 2020 services have been modified.  Appointments are done by telephone or a virtual visit (via OTN).  They will be doing some in-person appointments on a case-by-case basis.  Patients are advised to call the office to book an appointment.
 
In order to help reduce the spread of the virus, we ask for your help.  If you have respiratory symptoms (cough, fever, running nose, sore throat, difficulty breathing), have travelled outside of the country, or have had contact with someone with respiratory symptoms and travel, we ask that you STAY HOME, and call the Greenstone Family Health Team (during business hours) at 807-854-0051 for further direction or the Geraldton District Hospital (after hours) at 807-854-1862.  PLEASE DO NOT ATTEND OUR OFFICE WITHOUT INSTRUCTION IN THIS CASE.

Medical care team focused on family and community health
  • Offers timely access to primary care through Advanced Access (same-day bookings)
  • Provides access to other health care professionals within and outside of the Family Health Team
  • Coordinates documentation if patient receives care from a member of the team
  • Administers various health programs
  • Offers a walk-in medical clinic for urgent, primary health care concerns
Offers primary health care services including but not limited to:
  • Chronic disease education and prevention
  • Disease management
  • Health promotion
  • Palliative care
  • Rehabilitation

  • Must have a valid Ontario health (OHIP) card

  • Booked appointments required except for Walk-in Clinic

  • No referral required
