Description
  • Develops person-centred plans that identify the wants and needs of the individual requiring support
  • Provides a wide range of residential supports and services
  • Provides supports for individuals to make choices in becoming involved and connected with their community, on their own interests and dreams
  • Develops supports, in partnership with individuals and their families or others closest to them, around choices in terms of where they live, with whom and the level of support required

LifeShare
  • Offers support for individuals residing in a family setting with an associate family
  • Monitors, closely, the transition process of moving into an associate family
  • Encourages the individuals's natural family to maintain active involvement and to participate in the placement process
  • Maintains ongoing contact with the associate family, the individual, and the natural family

Supported Independent Living (SIL)
  • Provides support in a residential setting where 1-2 individuals live together in an apartment setting within the community
  • Focuses on assisting the individuals to live and participate in the community as independently as possible
  • Provides support based on the individual's needs
  • Provides specific supports such as meal and grocery planning, banking and budgeting, apartment maintenance, social and community networking, advocacy and rights, healthy living choices and other aspects of daily living
