Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services
  • Offers basic foot care
  • Coordinates prevention/educational activities such as Diabetes Lunch n Learn, and Diabetes Bingo
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals

Members of Wahnapitae First Nation living with diabetes or at-risk
