  • Provides diabetes services and supports including nutrition and weight management, consultation and on going treatment, insulin pump and continuous glucose monitor education
  • Delivers services through diabetes health professionals
  • Educates individuals with diabetes on how to self-manage their disease
  • Works closely with members of the community to meet the needs of individuals with diabetes within their area
