Home and Community Care Program


Description
Provides health care services for ill, injured or elderly community members to allow them to remain in their own homes

Provides the following services:
  • Assessment to determine individuals health history and develop a personalized care plan
  • Case management to ensure that care plans are being carried out and that the plan developed is still suitable
  • Home care through Personal Support workers
  • Personal care help with activities such as dressing and bathing
  • Basic foot care
  • Teaching the individual in need to care for themselves
  • Companionship
  • Home support with light housekeeping, laundry and meal preparation
  • Respite for family members and caregivers
  • Help with light maintenance and outdoor/yard work at Elder's homes
  • Non-emergency transportation for banking, shopping and other errands
Also coordinates the following programs:
  • Adult Day Program
  • Helpline
  • Meals on Wheels

Eligibility
  • Members of Serpent River First Nation who are seniors (60 years and older), frail elderly and/or people with disabilities who need physical, social, or emotional supports
  • Must be ambulatory

  • Intake assessment upon registration required
  • Accepts referrals from any source including self-referral
