Description
Medical care team focused on family and community health
  • Offers a multi-disciplinary health team of Physicians, Nurse Practitioners, Registered Nurses, Dietitians, Pharmacist, Chiropodist and Consulting Psychiatrist dedicated to providing quality primary care
  • Coordinates documentation if patient receives care from a member of the team
  • Provides access to other health care professionals within and outside of the Family Health Team

  • Anticoagulation (INR) Monitoring
  • Breast, Cervical and Colorectal Cancer Screening
  • Chiropody
  • Chronic Disease self-management
  • Cognitive Assessments
  • COPD/Spirometry
  • Diabetes Education
  • Hypertension Management
  • Lactation consulting
  • Medication Counselling and follow-up
  • Nutrition Counselling
  • Smoking Cessation
  • Social Work
Telephone Health Advisory Service
  • Provides access to a Registered Nurse who provides advice about urgent health care concerns
  • Offers the opportunity to have family doctor receive a written summary of the call
Eligibility
  • Must be a patient of a Sunset Country Family Health Network physician at the Paterson Medical Centre, Keewatin Medical Clinic or Lakeside Medical Office
  • Must have valid OHIP card to receive services
