- Description
- Provides services that promote physical and mental health for community members
- Coordinates health care services through a health team that consists of Community Health Nurse and Community Health Representative
- Coordinates Physician consultations weekly through visiting Physicians from Kenora and other locations
- Coordinates dentist appointments twice per month
- Provides health awareness and counselling, education and prevention workshops especially in the areas of diabetes and alcohol/drug abuse
- Coordinates clinics/workshops in areas that may include Diabetes, Foot Care, Immunization, Sexual Health and Communicable Diseases
- Publishes a monthly newsletter which is mailed out to area residents
- Advocacy on behalf of individual with other health professionals
- Coordination of one-on-one prenatal care with the community health nurse
- Home visits to the sick, elderly, new mothers and persons with health care needs
- Assistance with coordinating local and long distance travel arrangements for medical appointments
