- Application Process
- Referrals can be made through the Thunder Bay Mental Health Programs Intake Coordinator or a completed Thunder Bay Mental Health Program Referral Form can be submitted through the Access Point Northwest website
- Referrals are accepted from clients, physicians, and community professionals and agencies
- Some services require referral directly from a physician or nurse practitioner
