- Description
- Provides services that promote physical and mental health for community members
- Coordinates health care services through Community Health Nurse, Community Health Representative, Clerk and Diabetes worker as well as other professionals
- Provides health awareness and counselling, education and prevention workshops
- Coordinates clinics/workshops in areas that may include Immunization, Sexual Health, Smoking Cessation and Communicable Diseases
- Administers a Safe Needle Exchange program and functions as the safe needle exchange and safe inhalation kit pick-up and drop-off site
- Intake services to determine health care needs of individual
- Referrals to appropriate health care providers based on information in the intake assessment
- Advocacy and support on behalf of individual with other health professionals
- Coordination of one-on-one pre-natal care with the community health nurse
- Home visits to the sick, elderly, new mothers and persons with health care needs
- Assistance with coordinating local and long distance travel arrangements for medical appointments
- Annual Health Fair
- Children's Christmas celebration and 55+ Christmas celebration
