Description
A Housing First Program where workers provide outreach services to individuals/families assessed and determined to be chronically homeless

Supports Include:
  • Assistance in finding and maintaining housing, by reinforcing tenancy requirements such as payment of rent and utilities, and learning the skills necessary to maintain permanent housing
  • Orientation to an individuals surroundings and neighbourhood
  • Developing individual service plans to identify and address barriers
  • Assistance with accessing long term community services and supports
