Community Treatment Orders Program


Description
  • Provides alternatives to hospitalization by providing community treatment orders (CTO) to individuals who suffer from a serious mental disorder
  • Provides community based treatment, care, and supervision
  • Offers individualized support to help manage psychiatric illness and prevent relapse
Services include:
  • Mental health follow-up
  • Symptom monitoring and support
  • Assistance to obtain affordable housing
  • Crisis support
  • Financial assistance and budgeting support
  • Interpersonal support
  • Medication monitoring
  • Rehabilitation
  • Relapse prevention
Eligibility
Individuals aged 18 year of age and older, with serious mental illness who stabilize with treatment in hospital, and after discharge stop treatment, care and supervision resulting in relapse of illness and readmission to a psychiatric facility
Application Process
  • Referrals can be made by the individual, family, or health care professional
  • Call the office to make a referral
