- Description
- Provides services that promote physical and mental health for community members
- Provides health awareness and counselling, chronic disease management, education and prevention, especially in the areas of diabetes and alcohol/drug abuse
- Coordinates clinics/workshops in areas that may include Blood Pressure, Environmental Health, Immunization, Sexual Health and Communicable Diseases
- Advocacy on behalf of individual with other health professionals
- Coordination of pre-natal care and classes
- Home visits to the sick, elderly, new mothers and persons with health care needs
- Delivery of various health promotional programs such as the Annual Health Fair, Health Career Fair, Active Seniors and Diner's Club
- Assistance with coordinating local and long distance travel arrangements for medical appointments
