- Description
- Provides services that promote physical and mental health for community members and serves as the administration office for health and social services
- Coordinates health care services through visiting Physicians, Nurse Practitioners, Community Health Nurse, Dietitian and a Diabetes Team
- Provides health awareness and counselling, education and prevention workshops especially in the areas of diabetes and alcohol/drug abuse
- Coordinates clinics/workshops in areas that may include Diabetes, Foot Care, Immunization, Sexual Health and Communicable Diseases
- Offers a palliative care program when required
- Coordinates health initiatives/activities such as health fairs for different age groups
- Hosts recreation nights, women's socials and other events for community members
- Advocacy on behalf of individual with other health professionals
- Coordination of one-on-one prenatal/postnatal care with the community health nurse
- Home visits to the sick, elderly, new mothers and persons with health care needs
- Assistance with coordinating local and long distance travel arrangements for medical appointments
