Home and Community Care Program


Description
Provides health care services through Mnaamodzawin Health Services for ill, injured or elderly community members to allow them to remain in their own homes

Provides the following services:
  • Assessment to determine individuals health history and develop a personalized care plan
  • Case management to ensure that care plans are being carried out and that the plan developed is still suitable
  • Home care through Personal Support Workers
  • Personal care help with activities such as dressing and bathing
  • Basic foot care
  • Teaching the individual in need to care for themselves
  • Companionship
  • Home support with light housekeeping, laundry and meal preparation
  • Respite for family members and caregivers
Eligibility
Members of Sheshegwaning First Nation who are ill, injured or elderly (55 years and over)
Application Process
  • Intake assessment required
  • Accepts referrals from any source including self-referral
  • Valid health card and certificate of Indian status (if applicable) required
