- Description
- Offers a health centre providing family medical care, mental health services and many community-based health programs
- Coordinates health care services through Family Physicians, Nurse Practitioners, Community Health Nurses, Dietitian and a Diabetes Team and other health care professionals
- Provides health awareness and counselling, education and prevention workshops and seminars
- Coordinates clinics/workshops in areas that may include Diabetes, Foot Care, Immunization, Sexual Health and Communicable Diseases
- Offers a medicine lodge for culturally appropriate services through traditional healers
- Coordinates health initiatives/activities such as health fairs for different age groups
- Intake services to determine health needs of individual
- Coordination of one-on-one prenatal/postnatal care with the community health nurse
- Home visits to the sick, elderly, new mothers and persons with health care needs
- Assistance with coordinating local and long distance travel arrangements for medical appointments
- Advocacy on behalf of individual with other health professionals
- Outdoor trips in group settings for residential school survivors
- Parent involvement and resources to reduce the incidence of head lice in the schools
- Speech and language assessments, treatment and training with special needs workers
