Description
  • Provides timely access to family and community primary health care
  • Provides access to other health care professionals within and outside of the Family Health Team, such as a dietitian and a mental health counsellor
  • Operates a sexual health clinic that provides contraceptive information and sexual health care to both male and female patients of all ages
  • Coordinates documentation if patient receives care from a member of the team
  • Administers various health programs
Offers primary health care services including but not limited to:
  • Chronic disease education and prevention
  • Disease management
  • Health promotion
  • Palliative care
  • Rehabilitation
Specialty Services and Clinics available include:
  • Breast screening
  • Cardiac Rehab Program
  • Cervical Screening
  • Chronic Obstructive Pulmonary Disease (COPD) program
  • Colorectal cancer screening
  • Diabetes clinic
  • Geriatric Program
  • Hypertension (high blood pressure) clinics
  • Immunizations
  • Lifestyles Program
  • Mental health and social work programs
  • Pacemaker clinics
  • Smoking cessation programs
  • Stand Up Falls Prevention Program
  • Telederm program via Ontario Telemedicine Network (OTN)
  • Warfarin therapy management

Eligibility
  • Must be a resident of Wawa and surrounding area
  • Must have a valid Ontario health (OHIP) card
  • For new residents, call Wawa FHT directly to make arrangements to be rostered with a Physician

  • Requires booked appointments
  • No referral required for most services
