Perinatal Mental Health Program

Provides mental health services for mothers during pregnancy and up to 12 months after the baby is born
Services provided include:
  • Family assessment and support
  • Individual assessment and treatment
  • Referral to community resources
For individuals who have computer and printer access, a printable form can be downloaded and filled out to bring to the appointment. If not, the form is available for individuals when they go to their appointment.
Hours of Operation
Central Intake and Referral: Mon-Fri 8:30 am-4:30 pm
For mothers who are experiencing depression and anxiety during pregnancy and up to 12 months after the baby is born

Age Requirements: Ages: 16 year(s) and up;
Application Process
Self-referral, referral from community agencies, family doctor, obstetrician, midwife, or mental health professionals or through Central Intake and Referral at 705-523-4988 ext 4221
