Application Process
  • Physician's or nurse practitioner referral required
  • Referral by parents, educators, or therapists must be accompanied by a physician's referral

Service - Prescribed equipment ; None - Assistive Communication Clinic services ; Fee assistance available for prescribed equipment ; assistance is provided by Assistive Devices Program.
