Diabetes Education and Care Services

Search Agency Service

  • Specializes in the management of all types of diabetes
  • Teaches how to eat healthy and be more physically active
  • Teaches when and how to test blood sugar
  • Provides individual counselling
  • Offers diabetes presentations
  • Provides access to an endocrinologist (diabetes specialist) who conducts clinics on-site two days per week
Offers the following group classes:
  • Finding My Way With Diabetes
  • Diabetes Healthy Feet and You
Outpatient Insulin Start-up Program
  • Provides education, skills training and support to those who are starting insulin
Drop In Day Program
  • Offers service on a first-come, first-serve basis, no appointment is necessary
  • Answers urgent questions or concerns relating to diabetes
  • Offers access to a nurse, dietitian, and/or social worker for a brief, 15 minute visit
Children and Adolescent Program
  • Offers a specialized approach to meet the needs of families
  • Provides access to pediatrician, nurse, dietitian and social worker
  • Assists the family plan for school trips, a teenage party, sleeping-in and much more
Provides access to a variety of services including:
  • In-patient education programs
  • Education on the management of diabetes and related lifestyle issues
  • Assessment of home management and coping strategies
  • Support for the family, child and adolescent through visits, telephone contact and advocacy
  • Crisis management / intervention
Pregnancy Program
  • Assists in managing diabetes to help ensure a healthy pregnancy and baby
  • Offers pre-pregnancy counselling
First Step Walking Program
  • Helps individuals increase their physical activity
Insulin Pump Therapy
  • Offers an alternative to traditional insulin therapy
  • Provides a small device which is programmed to deliver insulin 24 hours a day in small amounts

Drop In Day Program Tue 1:30 pm-3 pm
Children and Adolescent Program:
  • Families with children/adolescents aged 0-19 who have diabetes

Pregnancy Program:
  • Women with gestational diabetes
  • Women who have diabetes and become pregnant
  • Diabetic women planning a pregnancy
Application Process
Outpatient Insulin Start-up Program:
  • A physician referral is required
  • Physician referral required
Program - First Step Walking Program; Subsidy is available to those who qualify
