Connects individuals with a Stroke Community Navigator

Helps individuals with:
  • Understanding the health care system, what services are available to them, and what services are best for them
  • Finding transportation, in-home nursing, personal care/housekeeping, physical activity programs
  • Coordinating with agencies to help with home modifications or permanent housing
  • Accompanies individuals to appointments, if required

Eligibility
Individuals who have suffered from a stroke

Individuals must:
  • Require physical assistance due to a permanent disability
  • Be medically stable
  • Be capable or potentially capable of directing their services
  • Be able to live safely unattended (assistance is by appointment only)


Age Requirements:16 years of age and older
