- Works in collaboration with the physicians and Nurse Practitioners at the Chapleau and District Family Health Team
- Provides a weekly drop-in visit for blood pressure checks, blood sugar checks, weigh-ins and healthy recipe ideas
Offers the following services:
- Blood sugar and blood pressure checks
- Education on diabetes
- Nutrition counselling specific to diabetes, high cholesterol, and high blood pressure
- Ordering of blood work
- Diabetic sensory foot screening
- Referrals to supports including chiropody and CNIB
- Diabetic retinopathy screening
- Eligibility
- Individuals living with diabetes or at-risk and their family members
