- Description
- Provides care and support to individuals experiencing a serious mental illness
- Focuses on maintaining or increasing an individual's ability to care for themselves, wellness planning, skill building, placement, and follow-up
- Supports individuals as they transition back into the community
- Offers skill development and education to individuals, families, and the community
- Offers outreach services to the individual, family members and community including ongoing support, education, skill development and development of the necessary community resources
- Eligibility
- Individuals 18 years of age and older must:
- Have a diagnosis of a serious mental illness
- Reside within the catchment area of the North Bay Regional Health Centre
- Be medically stable
- Have exhausted district level services and
- Have a signed letter of commitment from the referring agency/service
- Have a diagnosis of a serious mental illness
