Life Long Care Program


Description
Provides programs and services to address the needs of Aboriginal people who are either elderly or those with chronic illness and/or any form of disability

Indigenous Support
  • Advocates on a client's behalf to various agencies such as family physicians, landlords, Ontario Works, Ontario Disability Support Program, Canada Pension Plan and Old Age Security
Congregate Dining
  • Provides a weekly meal and activity
  • Provides transportation for attendees
Friendly Visiting
  • Arranges weekly visits from volunteers in an individual's home, Villa Minto, the hospital or at the Friendship Centre
Transportation Program
  • Provides transportation to various medical appointments
