Cochrane Timiskaming Branch - Community Support Program

Provides a variety of community based services to improve, maintain and/or restore stability to an individual living in the community
  • Provides ongoing assessments of independent functioning, daily living skills, and interpersonal skills
  • Assists individuals with the development of personal goals
  • Offers assistance with problem solving and personal development
  • Teaches independent living, functional, social and leisure skills
  • Coordinates services provided by other agencies and professionals
  • Provides support to individuals experiencing a mental health crisis
Case Management
  • Offers individuals the opportunity to participate in an intense recovery process focusing on changing attitudes, values, and feelings
Hours of Operation
Mon-Fri 8:30 am-4:30 pm
Individuals with a serious mental illness (e.g. main diagnoses of schizophrenia or bipolar disorder)
Case Management
  • Individuals receiving Community Support Services (services are designed to enhance and complement Community Support Services)
Application Process
Referrals are accepted from anyone but the individual must voluntarily accept and desire services
