  • Provides a variety of programs and services to meet the health care needs of the community
  • Offers assessment and treatment for non-life threatening injuries and illnesses
  • Provides referrals  to other local health programs and services
  • Provides coordination and intake for several housing programs under the Homelessness Network/Reseau des sans abri/Endaasgwok Maamwi-naadmaagewin

Eligibility
Francophone individuals (children and adults) who reside, work or study in the territory served by the Centre de santé communautaire du Grand Sudbury (CSCGS) and
  • Agree to communicate in French and to receive all primary care through CSCGS
  • Not currently receiving specialist health care for a condition that has not yet been diagnosed
  • Application Process
    • Registration forms available on website for new patients
    • Booked appointments required for most services
    Fees
    Most services covered by Ontario Health Insurance Plan (OHIP); Some fees for programs

    Valid Ontario health (OHIP) card or other proof of medical insurance coverage
