Description
Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services
  • Provides one-on-one or group consultation on monitoring blood sugar levels and medications
  • Offers basic foot care
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes including healthy eating, exercise, and other lifestyle choices
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals
  • Organizes activities such as Diabetes Bingo to raise awareness

Mon-Thu 8:30 am-4:30 pm, Fri 8:30 am-12 noon
Eligibility
Members of Henvey Inlet First Nation living with pre-diabetes, type 1 diabetes, and type 2 diabetes or at-risk
Application Process
  • Appointments recommended
  • No referral required
  • If transportation is required for any events, must make arrangements with the Health Centre 3 days prior to the event
