- Description
- Operates as a program under the Healing and Wellness Branch
- Provides information, advocacy and support to assist frail, vulnerable and at-risk elderly adults and individuals with care needs to remain in their homes as long as possible
- Acts as respite for informal caregivers such as extended family
- Caregiver support
- Case management
- Friendly visiting
- Service arrangement and coordination
- Transportation for medical appointments within Sudbury and/or assistance with medical appointment arrangements
- Visiting for social and safety reasons, either in person or on the phone
- Eligibility
- Focus is on frail, vulnerable and at-risk elderly adults and individuals trying to overcome barriers in accessing care such as low income, geographic isolation, language or disability issues or
- Any local, self-identified Métis resident who requires care in order to remain in their homes
- Application Process
- Intake assessment required
- Transportation arrangements must be booked 24 hours in advance; cancellation as soon as possible appreciated
