- Provides assisted living services to enable seniors to live independently in their own home
Offers the following services based upon the development of an individualized care plan:- Assistance with light housekeeping and laundry
- Aids with meal preparation
- Assists with personal care such as washing/bathing, dressing/undressing and toileting
- Medication reminders and monitoring
- Arranging appointments for medical and other services
- Assists clients with arrangements for rental contract and installation of lifeline equipment that enables them to obtain 24-hour emergency assistance
- Eligibility
- Individuals 60 years and older requiring assistance with daily living activities in order to remain in own home
- Individuals with health care needs requiring home assistance as determined by an assessment through Ontario Health atHome - North East Area
