COVID-19 Assessment Centre
  • ?The COVID-19 Assessment Centre is available by appointment ONLY to those who have been referred by the Northwestern Health Unit or Telehealth Ontario
  • It is recommended you complete the Ministry online Self-Assessment Tool at
  • Northwestern Health Unit can be reached at 807-468-7109 or the NHU Hotline 1-866-468-2240, the number for Telehealth Ontario is 1-866-797-0000
  • If you require an appointment, the Northwestern Health Unit will contact the COVID-19 Assessment Centre to identify your need for an appointment
  • The Assessment Centre will then contact you to schedule the appointment
  • The Assessment Centre will be in operation beginning Monday, March 30, 2020 and is open wekedays from 11 am-3 pm by appointment only
  • Assessment Centre is located on-site at the Lake of the Woods District Hospital (LWDH) (turn onto Nethercutt Dr and turn right onto Wolsely Ave - enter the assessment lane which will have a sign)
  • Once you arrive, read the instructions posted and ensure you follow them
  • Walk-ins at the Assessment Centre is strictly prohibited
Operated by Lake of the Woods District Hospital (LWDH) in partnership with the Sunset Country Family Health Team (SCFHT), Kenora District Services Board (KDSB), Waasegiizhig Nanaandawe'iyewigamig (WNHAC) and Kenora Chiefs Advisory (KCA), in close collaboration with the Northwestern Health Unit.
Hours of Operation
Assessment Centre Hours: Mon-Fri 11 am-3 pm - by appointment only
Individuals with:
* a new or worsening cough and/or fever over 38 degrees C, and
* in the past 14 days have either travelled outside of Canada or been in contact with someone who has a confirmed or probable case of COVID-19, or with someone who has symptoms of COVID-19 who recently travelled outside of Canada, or to an impacted area of Canada
