Description
  • Provides education through programs and services to individuals at-risk or living with chronic disease
  • Offers in-home and community-based services through Diabetes Educators including screening, testing and foot care

Diabetes and Chronic Disease Management services include:
  • Advanced foot care
  • Basic wound care
  • Chronic disease self-management programs
  • Community education sessions
  • Diabetes care plans
  • Diabetes self-management
  • Referrals to other programs and services
  • Screening (A1c, ACR, Blood glucose, Blood pressure, Monofilament)
Hours of Operation
Office Mon-Fri 9 am-5 pm
Eligibility
  • First Nation individuals at-risk or living with diabetes/chronic disease
  • A child residing off-reserve in the care of Dilico or a client receiving care or services of Dilico
  • Individuals receiving care or services with Dilico
Application Process
Written or verbal referrals screened by Diabetes Educator to determine eligibility
