Description
Provides support to individuals struggling with a combination of mental illness and abuse of alcohol or other substance (concurrent disorder)
  • Provides case management services
  • Provides a treatment plan based on a comprehensive assessment, focused on identifying treatment needs and issues in the areas of both substance use and mental health
  • Facilitates psychiatric consultation and follow-up
  • Provides family support and education
  • Assists individuals with concurrent disorder issues to navigate the complex system of community, health and social services
  • Offers aftercare services
