Description
Provides a monthly diabetes clinic which offers education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services
  • Offers basic foot care services
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Communicates patients' concerns to family physician
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals and the Northern Diabetes Health Network
Hours of Operation
First Tue of month: 9 am-4:30 pm
Eligibility
Individuals diagnosed with diabetes or high-risk
Application Process
Maintains list of diabetic patients; will contact individuals with date/time of next clinic
Fees
Service - Some tests and/or forms are not covered by Ontario Health Insurance Plan (OHIP) ; None
