Description
  • Teaches people living with chronic disease how to live better and tips that will help them improve their quality of life
  • Offers a free 6-week program with group sessions held for 2.5 hours weekly
  • Each participant receives a free copy of the Living a Healthy Life with Chronic Conditions Wellness Guide
  • Facilitated by Registered Nurse and community members living with chronic disease that can include but not limited to arthritis, fibromyalgia, diabetes, lung disease and heart disease
Hours of Operation
Office: Mon-Fri 8 am-4 pm
Clinic: Mon-Thu 9 am-4:30 pm, Fri 9 am-12 noon
Eligibility
Open to individuals living with any type of chronic disease including diabetes, arthritis, stroke, hypertension, lung disease, cancer or fibromyalgia
